All of the necessary liablity waivers and medical release forms can be obtained from this page. These forms cannot be submited to us electronicly as we must have your orginal signature(s). To access each waiver form, just click on the name of the form. You will need Adobe®Reader® to view and print the waivers. (In the unlikely circumstance that you don't have Adobe® Reader® on your machine, got to the Adobe® web site at www.adobe.com to download the free softwear.) Once the window opens with waiver, print using the print button in the window. When finished printing, use the back button on the window tool bar to return to this page. When printing the medical forms, use the print funtion in your browser, then use the back button at the bottom of the page to return to this page.

Liabillity Release Form: This form must be completed for each adult attending any of our sponsored activites, even if not riding. This only needs to be completed once a year. We will keep it on file for you.

Liability Release Form on Behalf of a Child: This must be completed by the parent or legal guardian of each child under the age of 18 attending our events. It CANNOT be completed by an other person; not a trainer, another parent or the person "responsible for the child" for the day. If the parent will NOT be attending the event, the signatures should be NOTARIZED, not simply witnessed.

Medical Release for a Child. This form needs to be completed by the parent or legal guardian of each child under the age of 18 when the parent or legal guardian will not be on the grounds. We also suggest that this be completed for each child even if the parent will be present. If the parent will not be present, this should be NOTARIZED, not simply witnessed.

Emergency Medical Information Card. This will assist us in getting you medical attention in the event you should come to grief. Please fill it out and carry on you person when you ride. We also ask that you give a copy to the Field Secretary. We will keep that copy in the hound/horse trailer, just in case you forget yours, or if it falls victim to sweat or the elements.
